Intake requirements
Intake requirements
Masks for intake/session are optional. If you would like your therapist to wear a mask. Please let them know. Masks are avaliable if needed. Please let me know your preference prior to your appt so that I may accommodate you.
A PDF intake form as well as a liability release form are available for print on this page. Please print, read, sign, date and bring them with you to your appt. If you can not print them copies will be provided at intake for you to complete.
If you are experiencing a fever, cough or any other symptoms of sickness please cancel your appt.
If you are immune compromised seek medical advice from your Dr. regarding massage.
